PPMNJ through the Gender Affirming Care program offers gender affirming hormone therapy (GAHT) using informed consent for patients 18 years of age and older, providing them with compassionate, validating, and thorough care. This includes medical transitioning as well as linking patients with additional resources to help them be their most authentic selves. In tandem with trained prescribing clinicians, Patient Navigator for Gender Affirming Care, Avery Heimann, (they/them pronouns) assists patients in accessing wraparound services, including a bimonthly virtual GAHT Support Group for patients in the program. As the Navigator for GAHT, Avery also works with patients to make sure their care is as affordable as possible.
